Overview

Host mucosal immune signaling pathways represent the integrated network of molecular signals that maintain homeostasis and coordinate defense at mucosal surfaces such as the gut and lungs (Mowat & Agace, 2014, Nature Reviews Immunology). These pathways are activated by pattern recognition receptors (PRRs) that detect microbial signals, triggering cascades like the NF-kB and JAK-STAT pathways to regulate cytokine production and cellular recruitment (Belkaid & Hand, 2014, Science). In healthy states, these signals ensure a balance between tolerance to commensal microbes and protective immunity against pathogens. However, dysregulation of these pathways is a primary driver of chronic inflammatory diseases, including Crohn's disease and ulcerative colitis, where excessive signaling leads to persistent tissue damage (Xavier & Podolsky, 2007, Nature). Modern therapeutics target specific nodes within these pathways, such as TNF-alpha, IL-23, or Janus kinases, to suppress pathological inflammation and restore the mucosal barrier (Danese et al., 2019, Lancet). While effective, broad modulation of these pathways can compromise host defense, leading to safety concerns regarding opportunistic infections and long-term immune surveillance.

Mechanism of action

Therapeutic agents modulate these pathways by neutralizing pro-inflammatory cytokines, blocking leukocyte trafficking to mucosal tissues, or inhibiting intracellular signaling transducers like Janus kinases (Neurath, 2014, Nature Reviews Immunology).
