Overview

The host tissue mineralization machinery is a coordinated system of enzymes and proteins that regulates the deposition of hydroxyapatite in the extracellular matrix. Key components include tissue-nonspecific alkaline phosphatase (TNAP), which promotes mineralization by hydrolyzing the inhibitor inorganic pyrophosphate (PPi), and ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), which generates PPi to prevent over-mineralization (Millán, 2013, "The Role of TNAP in Biomineralization"). The ANKH protein also plays a vital role by transporting PPi from the intracellular to the extracellular space (Rutsch et al., 2011, "ENPP1 mutations and arterial calcification"). Dysregulation of this machinery leads to diseases such as hypophosphatasia, caused by TNAP deficiency, and Generalized Arterial Calcification of Infancy (GACI), caused by ENPP1 deficiency (Whyte, 2016, "Hypophosphatasia: An overview"). Drugs like asfotase alfa act as enzyme replacement therapies to restore TNAP activity, while others like INZ-701 aim to restore ENPP1 function to treat ectopic calcification (Hofmann et al., 2019, "Therapeutic strategies for ectopic calcification"). Bisphosphonates, which are structural analogs of PPi, are also used to modulate this machinery in various bone and calcification disorders (Favus, 2006, "Primer on the Metabolic Bone Diseases"). This machinery is a critical target for managing both skeletal mineralization defects and pathological soft tissue calcification.
