Human anti-equine antibodies (HAEAs) are immunoglobulins produced by the human immune system in response to the administration of equine-derived therapeutic proteins, such as anti-thymocyte globulin (ATG), antivenoms, and antitoxins (Lawley et al., 1984, N Engl J Med). These antibodies, which include isotypes such as IgE and IgG (specifically IgG2 in response to carbohydrate moieties), represent a significant clinical complication known as immunogenicity (Bielory et al., 1988, J Allergy Clin Immunol). IgE-mediated HAEAs can trigger immediate type I hypersensitivity reactions, including life-threatening anaphylaxis, by causing mast cell degranulation upon re-exposure to equine proteins (Atgam Prescribing Information, FDA). IgG-mediated HAEAs, particularly when formed in high titers, lead to type III hypersensitivity or serum sickness, where immune complexes deposit in small blood vessels, joints, and kidneys, causing systemic inflammation (Vaughan et al., 1967, J Allergy). Furthermore, HAEAs can act as neutralizing antibodies, binding to the active sites of equine-derived drugs and significantly reducing their clinical effectiveness (Buchwald et al., 1987, Am J Med). Consequently, the presence of HAEAs is a major limiting factor in the use of equine biologics and requires careful patient monitoring and often premedication with immunosuppressants.
