Overview

The human corneal endothelium is a monolayer of highly specialized, hexagonal, terminally differentiated cells that line the posterior surface of the cornea, adjacent to the aqueous humor. These cells play a crucial physiological role in maintaining corneal hydration and clarity by regulating the fluid and solute transport between the aqueous humor and corneal stroma through a combination of a leaky barrier and ion pumps, principally Na^+/K^+-ATPase and carbonic anhydrase. The cells are of neural crest origin, are post-mitotic (do not readily divide to repair damage), and loss beyond a critical threshold leads to corneal swelling (edema) and visual impairment. They are a focus of regenerative cell therapies but are not a canonical druggable molecular target in the conventional sense.
