Overview

Human cytomegalovirus immediate-early 2 (IE2) mRNA is a primary transcript of the UL122 gene, essential for the initiation and progression of the HCMV lytic replication cycle (White et al., 2007, Journal of Virology). The mRNA encodes the IE2 protein, also known as IE86, which acts as a master transcriptional regulator that activates the expression of early and late viral genes while autoregulating its own promoter (Stenberg et al., 1990, Journal of Virology). Because the IE2 protein is indispensable for the production of infectious progeny, its mRNA has been successfully utilized as a therapeutic target for antisense intervention. The drug Fomivirsen is a phosphorothioate antisense oligonucleotide that specifically hybridizes to the IE2 mRNA, preventing translation through steric hindrance or RNase H-mediated degradation (Perry and Balfour, 1999, Drugs). This inhibition effectively halts viral replication, making it a critical target in treating CMV-related diseases, particularly in immunocompromised individuals. Clinical application of targeting this mRNA has primarily focused on CMV retinitis, where local suppression of the virus is necessary to prevent vision loss (Roehr, 1998, J Int Assoc Physicians AIDS Care).
