The human cytomegalovirus pp65 peptide-MHC class I complex consists of an antigenic peptide (derived from the highly immunodominant pp65 protein of HCMV) bound within the peptide-binding groove of a classical MHC class I molecule (commonly HLA-A2). This complex presents on the surface of infected cells and acts as a primary target for cytotoxic CD8+ T lymphocytes. Recognition of this complex by T cell receptors is crucial for antiviral immunity, making it a principal biomarker and therapeutic target in monitoring and controlling CMV infection, especially in immunocompromised patients and transplant recipients. The complex is also subject to CMV-mediated immune evasion tactics, such as MHC class I modulation and altered antigen processing, which can impact immune recognition and disease outcomes.
