The human extracellular matrix (ECM) is a complex, dynamic, and non-cellular network of macromolecules that fills the space between cells in all tissues and organs. It provides essential physical scaffolding for cellular constituents and initiates crucial biochemical and biomechanical cues required for tissue morphogenesis, differentiation, homeostasis, repair, and disease processes. The ECM is primarily composed of structural proteins (mainly collagens), glycoproteins (e.g., fibronectin, laminin), and proteoglycans (core proteins with glycosaminoglycan side chains such as heparan sulfate, chondroitin sulfate, keratan sulfate). It provides structural support, regulates cell behavior, mediates cell adhesion, stores growth factors, facilitates communication, and enables dynamic remodeling. Different tissues have specialized ECMs reflecting their mechanical needs. Cells interact with the ECM via surface receptors, including integrins, DDRs, and syndecans. ECM undergoes constant remodeling involving synthesis/degradation cycles critical for normal physiology and pathological states. MMPs are key enzymes responsible for controlled breakdown of ECM components.
