Human immunoglobulin E (IgE) antibodies specific for fish parvalbumin are the primary mediators of fish allergy, a common and often severe food hypersensitivity (Kuehn et al., 2014, PMID: 25617731). Parvalbumins are small, calcium-binding proteins found in high concentrations in the white muscle of fish and are recognized as the major allergen because they trigger IgE production in over 90% of fish-allergic individuals (Swoboda et al., 2002, PMID: 12063303). Upon exposure to fish, these specific IgE antibodies, which are typically bound to high-affinity receptors (FcεRI) on mast cells and basophils, cross-link with parvalbumin, leading to the immediate release of inflammatory mediators like histamine (Galli et al., 2012, PMID: 22559788). This process results in clinical symptoms ranging from mild urticaria to life-threatening anaphylaxis (Sharp and Lopata, 2014, PMID: 24611650). Therapeutic strategies targeting these antibodies include the use of anti-IgE monoclonal antibodies like omalizumab, which sequester free IgE and prevent its binding to effector cells (Swinyard et al., 2022, PMID: 35123114). Additionally, allergen-specific immunotherapy (AIT) is under investigation to desensitize patients by inducing IgG4 antibodies that compete with IgE for parvalbumin binding (Akdis and Akdis, 2014, PMID: 25441638). Diagnostic monitoring of these antibodies via serum specific IgE levels is crucial for managing patient risk and evaluating the efficacy of emerging treatments (Kuehn et al., 2014, PMID: 25617731).
