Overview

Hyaluronoglucosaminidase PH-20, also known as Sperm Adhesion Molecule 1 (SPAM1), is a glycosylphosphatidylinositol (GPI)-anchored enzyme primarily located on the sperm surface and inner acrosomal membrane. Its primary biological role is to facilitate fertilization by degrading the hyaluronan-rich cumulus cell layer surrounding the oocyte, allowing sperm penetration. In clinical practice, recombinant human PH-20 is utilized as a spreading factor to increase the dispersion and absorption of co-administered drugs, particularly in subcutaneous formulations of monoclonal antibodies. It has also been extensively studied as a therapeutic target in oncology, where it aims to deplete the dense hyaluronan matrix in the tumor microenvironment to enhance the delivery of chemotherapeutic agents. Despite promising early results, clinical trials for PEGylated versions in pancreatic cancer failed to meet primary survival endpoints. Beyond its role in reproduction and oncology, PH-20 is implicated in the inhibition of remyelination in central nervous system disorders like multiple sclerosis.
