Biofilms are composed of microorganisms embedded in a self-produced matrix of extracellular polymeric substances (EPS), which includes polysaccharides, proteins, and nucleic acids, all of which are highly hydrated (Flemming & Wingender, 2010). Necrotic tissue, or slough, consists of dead cellular debris that has lost its structural integrity and serves as a reservoir for pathogens and inflammatory mediators (Atkin, 2014). In clinical practice, these components are targeted for removal (debridement) to promote healing and reduce bacterial load. Physical modalities like Er:YAG lasers specifically target the water molecules (the primary hydrated component) within these structures to achieve precise tissue ablation through rapid vaporization (Walsh, 2003). This targeting is essential for managing conditions like diabetic foot ulcers and periodontitis, where the biofilm acts as a physical barrier to both antibiotics and the host immune system.
