HCN4 is a hyperpolarization-activated cyclic nucleotide-gated cation channel responsible for the pacemaker ("funny," If) current in cardiac sino-atrial node cells, essential for spontaneous diastolic depolarization and heartbeat generation. It is permeable to both sodium and potassium ions, activated by membrane hyperpolarization, and modulated by intracellular cyclic nucleotides (especially cAMP). HCN4 is most highly expressed in heart tissue, but also found in the nervous system and embryonic cells. Mutations in HCN4 are associated with various cardiac arrhythmias and developmental disorders. HCN4 is the therapeutic target for ivabradine, a drug used to lower heart rate in angina and heart failure patients. Selectivity and safety remain key challenges in clinical applications due to structural similarity among HCN channel subtypes and broad tissue distribution.
