Overview

Hypersulfated heparan sulfate glycosaminoglycans (HS-GAGs) are complex, highly acidic polysaccharides found on the cell surface and within the extracellular matrix (1, 3). They are characterized by a high density of sulfate groups, which create specific "sulfation codes" that allow them to interact with a vast array of proteins, including growth factors, chemokines, and morphogens (3, 8). These molecules act as essential co-receptors, modulating signaling pathways critical for cell proliferation, migration, and tissue homeostasis (6, 10). In pathological conditions, hypersulfated HS-GAGs play a pivotal role in stabilizing amyloid fibrils in systemic amyloidosis and Alzheimer's disease, and they serve as primary attachment sites for viruses like SARS-CoV-2 and Herpes Simplex Virus (2, 4, 13). Furthermore, they are involved in cancer progression by facilitating angiogenesis and tumor metastasis (9). Therapeutic strategies targeting these molecules include the use of peptide-antibody fusions like zamubafusp alfa (AT-02) to promote the phagocytic clearance of amyloid deposits, as well as heparin mimetics that competitively inhibit protein-GAG interactions (11, 15).
