"Hypotensive" is a physiological and pharmacological descriptor used to characterize a state of low blood pressure (hypotension) or the property of a substance to reduce arterial pressure. It is not a canonical name for a specific molecular target, such as a single protein, enzyme, or receptor. Instead, the term refers to a functional class of "hypotensive agents" (antihypertensives) that interact with diverse biological pathways to achieve vasodilation or reduced cardiac output. Historically, some endogenous molecules, most notably Adrenomedullin (ADM), were referred to as "hypotensive factors" during their initial discovery and characterization. Hypotensive effects are the primary therapeutic goal of antihypertensive medications, which target diverse molecules including the angiotensin-converting enzyme (ACE), calcium channels, and various G protein-coupled receptors like the adrenergic and imidazoline receptors. In clinical practice, hypotension can also occur as an adverse side effect of many drug classes, leading to concerns such as syncope or organ hypoperfusion. Because it refers to a clinical effect or category rather than a discrete biological entity, "Hypotensive" is considered an incorrect term for a specific molecular target.
Hypotensive agents act through various pathways to reduce blood pressure, including the inhibition of the renin-angiotensin-aldosterone system (RAAS), blockade of L-type calcium channels, activation of potassium channels, or stimulation of vasodilatory receptors such as the imidazoline I1 or adrenergic alpha-2 receptors.
