Overview

The idiotype region of tumor-derived immunoglobulin refers to the unique antigenic determinants found within the variable (V) regions of immunoglobulin molecules expressed by tumor (especially B cell tumor) clones. These idiotype determinants (also called idiotopes) are generated by unique combinations of gene rearrangements and somatic mutations found in each malignant clone’s immunoglobulin. The idiotype acts as a true tumor-specific antigen, making it a compelling, highly individualized target for immunotherapy in diseases such as non-Hodgkin lymphoma and multiple myeloma. Therapeutic strategies exploit the idiotype’s uniqueness through personalized vaccines (using the idiotype protein, peptide, or gene), anti-idiotype monoclonal antibodies, or idiotype-pulsed dendritic cells to stimulate an immune response against the malignant B cells. The idiotype’s role in immune regulation is also central to Jerne’s network theory of immunity, in which anti-idiotype antibodies can mimic the original antigen and profoundly influence immune network dynamics. Tumor-derived idiotypes may contribute to immune evasion, and abnormal glycosylation or immune responses to these regions can influence cancer progression. The need for individualized therapy and possible risks of tolerance or regulatory immune responses are notable therapeutic challenges.
