Overview

The immune and inflammatory milieu refers to the complex, dynamic environment composed of various immune cells, cytokines, chemokines, and signaling molecules that orchestrate the body's response to injury, infection, or malignancy (Nature Reviews Immunology, 2021). It is not a single molecular target but rather a collective biological state that influences disease progression and therapeutic outcomes (Frontiers in Immunology, 2020). In conditions like cancer, the milieu can be immunosuppressive, protecting tumors from the immune system through the recruitment of regulatory T cells and the secretion of factors like TGF-beta (NCBI, 2022). Conversely, in autoimmune diseases, the milieu is characterized by an overabundance of pro-inflammatory factors such as TNF-alpha and IL-6, which drive chronic tissue destruction (StatPearls, 2023). Therapeutic strategies often aim to modulate this environment by targeting specific nodes within the network, such as using monoclonal antibodies to neutralize cytokines or checkpoint inhibitors to reinvigorate exhausted T cells (PubMed, 2021). Understanding the composition of this milieu is critical for developing precision medicines that can shift the environment from a pathological state back to homeostasis (NIH, 2023).
