Overview

Allogeneic hematopoietic stem cell engraftment is a clinical procedure wherein hematopoietic stem cells from a genetically matched (or partially matched) donor are infused into a recipient after conditioning therapy. The aim is to eradicate a diseased or defective hematopoietic/immune system and allow donor stem cells to repopulate the marrow, restoring normal blood cell and immune system production. This process underlies the curative potential of allogeneic HSCT for a range of malignant, autoimmune, and inherited diseases but is associated with significant risks, including graft-versus-host disease and infections.

Mechanism of action

Donor stem cell engraftment repopulates recipient marrow and regenerates immune system. Graft-versus-tumor/host responses involve immunologic effects of donor lymphocytes against malignant or host cells. The process also includes the removal of autoreactive or defective immune components via conditioning and cell replacement.
