An immunocompetent cell is a cell that has the capacity to develop an immune response following exposure to an antigen (StatPearls, 2023). This category primarily includes lymphocytes such as T cells, B cells, and natural killer (NK) cells, as well as professional antigen-presenting cells like dendritic cells and macrophages (NCI Dictionary, 2024). The primary biological function of these cells is to maintain host defense by identifying and neutralizing foreign pathogens and performing immunosurveillance against malignant cells (Abbas et al., Cellular and Molecular Immunology, 2021). Dysregulation of immunocompetent cells is a hallmark of autoimmune diseases, where they attack self-tissues, and chronic inflammatory conditions (PubMed, PMID: 31434123). While the term refers to a broad cell population rather than a specific molecular target, these cells are the primary site of action for many immunomodulatory and immuno-oncology therapies. Drugs interact with these cells by targeting specific surface receptors or signaling pathways, such as PD-1 on T cells or CD20 on B cells, to either enhance or suppress immune activity (Nature Reviews Drug Discovery, 2020). Monitoring the status and function of these cells is critical for assessing immune health and treatment efficacy in clinical settings.
