Immunoglobulin E (IgE) specific for Bet v 1 epitopes is the primary immunological driver of birch pollen allergy, a common type I hypersensitivity condition. Bet v 1 is the major allergen found in birch pollen (Betula verrucosa), and individuals sensitized to this protein produce specific IgE antibodies that bind to its various epitopes (UniProt P15494). These IgE molecules then attach to high-affinity FcεRI receptors on the surface of mast cells and basophils. When the individual is re-exposed to birch pollen, Bet v 1 cross-links the receptor-bound IgE, triggering the degranulation of these cells and the release of inflammatory mediators such as histamine and leukotrienes (Gould & Sutton, Nature Reviews Immunology, 2008). This process results in clinical manifestations including allergic rhinitis, conjunctivitis, and allergic asthma, as well as oral allergy syndrome due to cross-reactivity with homologous proteins in certain fruits and vegetables (Vieths et al., Annals of the New York Academy of Sciences, 2002). Therapeutic interventions targeting this pathway include allergen-specific immunotherapy (AIT), which seeks to induce immune tolerance and reduce the levels of Bet v 1-specific IgE while increasing protective IgG4 antibodies (Biedermann et al., Allergo Journal International, 2019). Monoclonal antibodies like omalizumab also interact with this target by binding to the Fc region of all circulating IgE, thereby preventing its attachment to effector cells and mitigating the allergic cascade (Xolair FDA Label).
Sequestration of circulating IgE to prevent binding to high-affinity FcεRI receptors on mast cells and basophils, or induction of immune tolerance through allergen-specific immunotherapy to decrease specific IgE titers and increase neutralizing IgG4 antibodies.
