Immunoglobulin E (IgE) specific for bluefish parvalbumin is a specialized antibody that mediates allergic reactions to the bluefish (Pomatomus saltatrix). The primary allergen involved is parvalbumin, a calcium-binding protein found in fish muscle tissue, which is designated as Pom s 1 in this species (WHO/IUIS Allergen Nomenclature). In sensitized individuals, these specific IgE antibodies are produced by B cells and reside on the surface of mast cells and basophils via the high-affinity FceRI receptor. Upon ingestion or inhalation of bluefish proteins, the allergen cross-links the IgE-FceRI complexes, triggering the rapid degranulation and release of inflammatory mediators like histamine and leukotrienes (StatPearls, 2023). This biological cascade results in clinical symptoms of fish allergy, ranging from mild urticaria to life-threatening anaphylaxis. While therapeutic agents like omalizumab target the constant region of all IgE molecules to neutralize them, specific IgE levels are primarily utilized as diagnostic biomarkers to assess sensitization and the risk of severe reactions (PubMed, PMID: 26022875). Understanding the cross-reactivity between bluefish parvalbumin and other fish parvalbumins is essential for the clinical management of polysensitized patients.
Anti-IgE monoclonal antibodies bind to the C-epsilon-3 (Ce3) domain of free circulating IgE, preventing it from binding to the high-affinity IgE receptor (FceRI) on mast cells and basophils, thereby inhibiting the release of inflammatory mediators.
