Immunoglobulin E (IgE) specific for cod proteins is a class of antibodies that mediates type I hypersensitivity reactions to Atlantic cod (Gadus morhua). The primary allergen recognized by these antibodies is Gad m 1, a parvalbumin protein that is highly stable and resistant to heat and digestion (Swoboda et al., 2002, Journal of Immunology). In sensitized individuals, these IgE antibodies are primarily found bound to high-affinity FcεRI receptors on the surface of mast cells and basophils. Upon subsequent ingestion of cod, the allergen cross-links these surface-bound antibodies, triggering the immediate release of inflammatory mediators like histamine and leukotrienes (Gould & Sutton, 2008, Nature Reviews Immunology). This target is clinically significant for both diagnosis and therapy; for instance, the monoclonal antibody omalizumab (Xolair) is used to sequester circulating IgE and reduce the risk of severe allergic reactions (Wood et al., 2024, New England Journal of Medicine). Monitoring the levels of cod-specific IgE, often identified by the diagnostic code f3, helps clinicians assess the severity of the allergy and the likelihood of clinical reactivity (Sicherer & Sampson, 2018, Journal of Allergy and Clinical Immunology). Therapeutic strategies also include allergen-specific immunotherapy, which aims to desensitize the patient by modulating the immune response from an IgE-mediated to an IgG4-mediated profile.
