Immunoglobulin E (IgE) specific for peanut allergens is the primary mediator of the rapid-onset allergic immune response in sensitized individuals. These antibodies are produced by B cells and plasma cells in response to specific peanut proteins, such as Ara h 1, Ara h 2, and Ara h 3, which are recognized as foreign threats (Sampson et al., 2018, J Allergy Clin Immunol). Peanut-specific IgE circulates in the blood and binds with high affinity to FcεRI receptors on the surface of mast cells and basophils. When a sensitized individual is exposed to peanuts, the allergens cross-link these surface-bound IgE molecules, triggering the explosive release of inflammatory mediators like histamine and leukotrienes, which can lead to life-threatening anaphylaxis (Galli & Tsai, 2012, Nature Medicine). On B cells, membrane-bound IgE serves as a component of the B-cell receptor, facilitating allergen uptake and the maintenance of allergic memory. Therapeutic strategies targeting this molecule include anti-IgE monoclonal antibodies that sequester free IgE to prevent mast cell priming and allergen-specific immunotherapy designed to desensitize the immune system by shifting the balance toward protective IgG4 antibodies (Wood et al., 2024, NEJM).
Monoclonal antibodies like Omalizumab bind to the Cε3 domain of circulating IgE, preventing its interaction with the high-affinity FcεRI receptor on mast cells and basophils. This sequestration reduces the density of IgE-FcεRI complexes on the cell surface and subsequently downregulates receptor expression, raising the threshold for allergen-induced activation (Wood et al., 2024, NEJM). Other agents like Quilizumab target the M1-prime segment of membrane-bound IgE on B cells to deplete IgE-switched B cells and prevent the production of new peanut-specific IgE (Gauvreau et al., 2014, J Allergy Clin Immunol).
