Immunoregulatory effect refers to the modulation of the immune system's response, encompassing both the enhancement (immunostimulation) and suppression (immunosuppression) of immune activity (StatPearls, 2023). It is a physiological or pharmacological outcome rather than a specific molecular target like a receptor or enzyme. In therapeutic contexts, achieving an immunoregulatory effect is the primary goal for treating autoimmune diseases, allergies, and preventing organ transplant rejection (NIH, 2022). Because the term describes a broad biological process or a drug's functional result, it does not represent a discrete protein or gene product that can be targeted by a ligand (IUPHAR/BPS Guide to Pharmacology). Drugs such as monoclonal antibodies, calcineurin inhibitors, and corticosteroids are used to induce these effects by acting on specific, well-defined molecular targets within the immune signaling cascade. Consequently, while the term is used to describe the desired clinical result, it lacks the molecular specificity required for target-based drug discovery. Analysts should distinguish between this functional outcome and the underlying molecular entities, such as cytokines or checkpoints, that serve as the actual therapeutic targets (PubMed, 2021).
