The incompetent superficial venous system refers to a pathological state rather than a specific molecular target. It is characterized by the failure of venous valves within the superficial veins, such as the great or small saphenous veins, which leads to the retrograde flow of blood known as venous reflux (StatPearls, 2023). This condition results in chronic venous hypertension, which is the primary driver for the development of varicose veins and more severe forms of chronic venous insufficiency (CVI), including skin changes and venous ulcers (Mayo Clinic, 2023). While it is an anatomical and physiological system, it is the focus of significant therapeutic intervention. Pharmacological treatments include sclerosing agents that chemically ablate the vessel to redirect blood flow to healthy deep veins, and venoactive drugs that target the inflammatory processes and loss of vascular tone associated with the disease (NIH, 2022). Because this entry describes a clinical condition and anatomical system rather than a protein, receptor, or enzyme, it is not classified as a traditional molecular therapeutic target.
Sclerosing agents (e.g., polidocanol) work by inducing local chemical damage to the vascular endothelium, leading to vessel thrombosis, fibrosis, and eventual permanent occlusion of the incompetent vein (StatPearls, 2023). Venoactive drugs (e.g., MPFF) aim to increase venous wall tension and reduce capillary permeability and inflammation (PubMed, 2021).
