Overview

Infected or inflamed gingival tissue is the soft tissue of the gums in a state of active inflammation and often bacterial infection, typically seen in gingivitis or periodontitis. Clinically, it is characterized by redness, swelling, bleeding, and sometimes pain, resulting from an immune response to bacterial biofilms (dental plaque) adhering to the teeth. Inflammation results in cellular infiltration (neutrophils, lymphocytes, macrophages), tissue remodeling, and, if persistent, progressive destruction of the periodontal support structures. Untreated, this condition can precede or progress to periodontitis, potentially leading to tooth loss and may have systemic health implications including increased risk of cardiovascular disease, adverse pregnancy outcomes, and associations with neurodegenerative disorders.

Mechanism of action

Reduction of bacterial load (antibacterial agents/antiseptics disrupt plaque and bacterial biofilm); Immunomodulation (anti-inflammatories reduce tissue response).

03

Biological functions

Immune response (inflammation with neutrophil and lymphocyte recruitment)Tissue remodeling (matrix/collagen breakdown and repair)Host–pathogen interaction (immune response to bacterial biofilm/plaque)
04

Disease associations

Inflammation (primary pathological process)Infection (bacterial, especially by dental plaque organisms such as Porphyromonas gingivalis)Other (contributes to periodontitis, tooth loss, and systemic complications such as risk for Alzheimer’s disease and cardiovascular conditions)
05

Safety considerations

Tissue destruction can progress to periodontal disease and tooth loss if not managedSystemic associations: risk of cardiovascular disease, adverse pregnancy outcomes, possible links to Alzheimer's diseaseAntibiotic resistance (with systemic antibiotic use)
06

Interacting drugs

Antibacterial mouthwashes (e.g., chlorhexidine)
