The inferior medial genicular nerve (IMGN) is a terminal sensory branch typically arising from the tibial nerve that provides nociceptive innervation to the inferomedial aspect of the knee joint capsule and the medial meniscus (StatPearls, 2023). It travels along the superior border of the popliteus muscle and courses around the medial condyle of the tibia, passing deep to the medial collateral ligament. While it is an anatomical structure rather than a molecular receptor or enzyme, it is a primary clinical target for interventional pain management, including genicular nerve blocks and radiofrequency ablation (RFA) (Choi et al., 2011). These interventions are standard for managing chronic knee pain in patients with advanced osteoarthritis or persistent pain following total knee arthroplasty. Pharmacologically, the nerve's signaling is modulated using local anesthetics like bupivacaine, which block voltage-gated sodium channels, thereby preventing the transmission of pain signals to the central nervous system. Precise anatomical targeting is required to ensure efficacy and avoid injury to the adjacent inferior medial genicular artery.
