The inflammasome is a cytosolic multiprotein complex that plays a central role in the innate immune system by detecting pathogenic microorganisms and cellular stress, thereby initiating inflammatory responses and programmed cell death. It acts as an intracellular sensor platform, assembling in response to pathogen-associated molecular patterns (PAMPs), damage-associated molecular patterns (DAMPs), or homeostatic disruptions. The canonical inflammasome typically consists of a sensor protein (usually a pattern recognition receptor (PRR) such as NLRP3, AIM2, NLRC4, or pyrin), an adaptor protein (apoptosis-associated speck-like protein containing a CARD (ASC)), and an effector protease (pro-caspase-1). Several types of inflammasomes have been identified based on their PRR component, including NLRP3, AIM2, NLRC4, and Pyrin inflammasomes. Upon detection of PAMPs/DAMPs by the sensor PRR, the sensor oligomerizes and recruits ASC, which then recruits pro-caspase-1, leading to autocatalytic activation of caspase-1. Activated caspase-1 cleaves pro-inflammatory cytokines IL-1β and IL-18 into their mature forms for secretion and also cleaves gasdermin D (GSDMD), whose N-terminal fragment forms pores in the cell membrane leading to pyroptosis, facilitating cytokine release and further inflammation. Aberrant activation or dysregulation contributes significantly to various autoinflammatory conditions. Targeting specific components—especially NLRP3—with small molecules or biologics has shown promise for treating chronic inflammatory diseases by blocking excessive inflammation without broadly suppressing immunity.
