Inflammatory cytokines are a diverse group of signaling proteins, such as Tumor necrosis factor-alpha (TNF-α), Interleukin-1 (IL-1), and Interleukin-6 (IL-6), that serve as critical mediators of the innate and adaptive immune responses (StatPearls, 2023). These cytokines exert their effects by binding to specific transmembrane receptors, which subsequently activate complex intracellular signaling pathways, most notably the Janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway and the nuclear factor kappa-light-chain-enhancer of activated B cells (NF-κB) cascade (NIH, 2022). Chronic overproduction or dysregulation of these molecules is central to the pathogenesis of various inflammatory and autoimmune disorders, including rheumatoid arthritis, psoriasis, and inflammatory bowel disease (PubMed, 2021). Therapeutic strategies often involve the use of biologic agents, such as monoclonal antibodies that neutralize cytokines or block their receptors, as well as small-molecule inhibitors targeting downstream signaling kinases (Nature Reviews Drug Discovery, 2020). While these interventions are highly effective at reducing systemic inflammation, they are associated with significant safety concerns, primarily an increased susceptibility to opportunistic infections due to broad immunosuppression (Frontiers in Immunology, 2021).
Therapeutic agents modulate this system by neutralizing circulating pro-inflammatory ligands, competitively inhibiting their cognate cell-surface receptors, or blocking intracellular signaling transducers such as Janus kinases (JAKs) to prevent the transcription of inflammatory genes.
