Inflammatory cytokines are a diverse group of signaling proteins, including Tumor Necrosis Factor-alpha (TNF-α), Interleukin-1 (IL-1), and Interleukin-6 (IL-6), that are primarily secreted by immune cells to coordinate the body's response to infection and injury (Zhang & An, 2007, PMID: 18220775). These molecules function by binding to specific cell-surface receptors, initiating intracellular signaling cascades such as the NF-κB and JAK/STAT pathways, which lead to the upregulation of inflammatory genes (Dinarello, 2000, PMID: 10985772). While crucial for acute host defense, the chronic or excessive production of these cytokines is a central driver in the pathogenesis of autoimmune disorders like rheumatoid arthritis and systemic inflammatory conditions (Feldmann & Maini, 2003, PMID: 12646911). Therapeutic intervention typically involves the use of monoclonal antibodies or receptor antagonists to neutralize cytokine activity, thereby reducing tissue damage and systemic inflammation (Taylor et al., 2004, PMID: 15128058). However, because these processes are vital for immune surveillance, their inhibition can lead to significant safety concerns, most notably an increased susceptibility to opportunistic infections (Singh et al., 2011, PMID: 21328444).
Drugs targeting inflammatory processes typically act through the direct neutralization of soluble cytokines (e.g., anti-TNF monoclonal antibodies), competitive inhibition of cytokine receptors (e.g., IL-1 receptor antagonists), or the inhibition of downstream intracellular signaling enzymes like Janus kinases (JAKs) to prevent the transcription of pro-inflammatory mediators (O'Shea et al., 2013, PMID: 23584302).
