The "Inflammatory pathways – inferred, not experimentally confirmed for XZZY" refers to a complex network of biological signaling processes hypothesized to be the primary therapeutic focus of the Xiaozhen zhiyang (XZZY) decoction [3, 4]. XZZY is a Traditional Chinese Medicine (TCM) formula composed of eight herbs, including Lonicerae japonicae flos and Sophorae flavescentis radix, specifically used to treat dermatological toxicities such as acneiform rash in patients receiving epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) [4, 7]. Through network pharmacology and gene enrichment analysis, researchers have inferred that the decoction modulates pathways related to inflammation, oxidative stress, and EGFR signaling, although these interactions lack comprehensive experimental confirmation for each of the formula's 103 potential targets [3, 4]. The decoction is thought to work by inhibiting the release of pro-inflammatory cytokines like TNF-alpha and IL-6, thereby reducing the skin inflammation and itching associated with targeted cancer therapies [4]. Clinical studies have demonstrated that topical application of XZZY significantly improves patient quality of life and reduces rash severity, as measured by the WoMo score [4, 9]. Despite its clinical efficacy, the complexity of the multi-herb formula and the reliance on computational inference for its mechanism of action present challenges for its pharmaceutical standardization and regulatory approval [7].
The decoction is thought to modulate pro-inflammatory cytokine release and regulate EGFR-related signaling cascades to mitigate skin toxicity induced by tyrosine kinase inhibitors.
