Overview

Innate and adaptive immune pathways stimulated by QS-21 Stimulon refer to the immunological mechanisms activated by the saponin adjuvant QS-21, a purified fraction from the bark of Quillaja saponaria (Agenus Inc., 2024). These pathways involve the activation of the NLRP3 inflammasome through lysosomal destabilization, which triggers the release of pro-inflammatory cytokines IL-1β and IL-18 (Welsby et al., 2016). This innate signaling cascade is crucial for the subsequent induction of Th1-type adaptive immunity and the enhancement of CD8+ T-cell cross-presentation (Didierlaurent et al., 2017). QS-21 facilitates the delivery of antigens into the cytosol of dendritic cells, allowing for the activation of cytotoxic T-lymphocytes (CTLs), which is a key requirement for effective vaccines against intracellular pathogens and cancer (Lacaille-Dubois, 2019). While highly effective at boosting vaccine potency, these pathways also contribute to the local and systemic reactogenicity associated with saponin-based adjuvants. Formulation with cholesterol is typically required to mitigate the hemolytic activity of QS-21 while preserving its potent immunostimulatory properties.

Mechanism of action

QS-21 stimulates immune pathways by inducing cholesterol-dependent endocytosis and lysosomal membrane permeabilization in dendritic cells, which activates the NLRP3 inflammasome and promotes the cross-presentation of antigens to CD8+ T-cells (Welsby et al., 2016; Didierlaurent et al., 2017).
