Overview

Inorganic pyrophosphate (PPi) is a small inorganic polyphosphate that serves as a critical endogenous inhibitor of extracellular mineralization (PubChem CID 1012). It is primarily generated from extracellular ATP by the enzyme ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1) and is degraded into inorganic phosphate by tissue-nonspecific alkaline phosphatase (TNAP) (NCBI, PMC4924461). In healthy individuals, PPi prevents the formation of hydroxyapatite crystals in soft tissues, such as blood vessels and skin. Deficiencies in PPi, often caused by mutations in ENPP1 or ABCC6, lead to severe calcification disorders like Generalized Arterial Calcification of Infancy (GACI) and Pseudoxanthoma Elasticum (PXE) (Inozyme Pharma). Conversely, excessive accumulation of PPi, as seen in Hypophosphatasia (HPP), inhibits normal bone mineralization, leading to rickets or osteomalacia (StatPearls, NBK499863). Therapeutic approaches include the use of PPi analogs (bisphosphonates), enzyme replacement therapies to restore PPi production (e.g., INZ-701), or direct PPi supplementation to manage ectopic calcification.
