Overview

The insular cortex is a distinct lobe of the cerebral cortex situated deep in the lateral sulcus. It is involved in integrating sensory, affective, and cognitive information, supporting processes such as interoception, emotional awareness, cognitive control, empathy, and homeostatic regulation[1][3][5][6]. The insula can be divided into anterior and posterior regions, each with specific functional roles, and is notable for its heterogeneous cytoarchitecture and complex connectivity with other brain regions[1][2][6]. Dysfunction or damage to the insular cortex is associated with a variety of neuropsychiatric and neurological disorders, including affective disorders, pain syndromes, and epilepsy[3][6][7]. The insular cortex is a brain region, not a molecule or receptor. It cannot be targeted in the same sense as typical drug targets (e.g., proteins, receptors). Any therapeutic approaches would involve modulating its activity indirectly (for example, via CNS drugs, neurostimulation, or behavioral therapies), but molecular targeting terms such as interacting drugs and mechanisms of action do not apply[3][6].
