Insulin B chain-reactive T cells are a specialized population of autoreactive lymphocytes that play a central role in the pathogenesis of Type 1 Diabetes (T1D) (NIH, 2020). These cells, primarily CD4+ helper T cells, possess T-cell receptors (TCRs) that specifically recognize peptide fragments from the insulin B chain, most notably the B:9-23 epitope, when presented by specific MHC class II molecules such as HLA-DQ8 in humans or I-Ag7 in mice (MedChemExpress; Frontiers in Immunology, 2022). Upon activation, these cells infiltrate the pancreatic islets (insulitis) and orchestrate the destruction of insulin-producing beta cells through the secretion of pro-inflammatory cytokines like IFN-gamma and the recruitment of cytotoxic CD8+ T cells (FierceBiotech, 2020; MedChemExpress). Because of their pivotal role in initiating and driving the autoimmune attack, they are a primary target for antigen-specific immunotherapies (NIH, 2020). Therapeutic approaches include the administration of insulin peptides or DNA vaccines to induce immune tolerance, as well as the development of engineered CAR-T or CAR-Treg cells to selectively eliminate or suppress these pathogenic clones while sparing the rest of the immune system (Harvard, 2020; NIH, 2024). Current clinical strategies also involve broader T-cell modulators like Teplizumab, which aim to preserve beta-cell function by interfering with T-cell activation and exhaustion (NIH, 2020).
