INS-IGF2 is a readthrough chimeric transcript joining the insulin (INS) gene on the 5′ end with the IGF2 gene on the 3′ end. The resulting protein shares the N-terminus with insulin and exhibits a unique, extended C-terminus derived from IGF2 noncoding exons, distinct from prepro-IGF2. Two transcript isoforms exist: one is predicted to encode a stable chimeric protein, while another is a candidate for nonsense-mediated decay. The locus is imprinted and paternally expressed in limb and eye tissues. INS-IGF2 expression is detectable in insulinomas, with evidence that it may be recognized by autoantibodies in type 1 diabetes, potentially enhancing insulin autoantigenicity. Its precise physiological and pathological roles remain unclear, and there is currently no evidence of use as a therapeutic target or as a target of known drugs.
