Overview

The Insulin-like growth factor 1 receptor (IGF1R) is a transmembrane receptor tyrosine kinase that plays a critical role in cardiac physiology and pathophysiology. In cardiomyocytes, IGF1R signaling is essential for normal heart development, postnatal growth, and the maintenance of cardiac structure and function [1, 2]. Activation of the receptor by its ligand, IGF-1, triggers downstream pathways such as PI3K/Akt and MAPK/ERK, which promote cardiomyocyte survival, protein synthesis, and physiological hypertrophy [3, 4]. While IGF1R signaling is generally cardioprotective, its dysregulation is linked to various cardiovascular diseases, including heart failure and diabetic cardiomyopathy [5]. Therapeutic targeting of IGF1R is primarily explored in oncology to inhibit tumor growth, but such interventions require careful monitoring for potential adverse effects on cardiac health due to the receptor's vital role in the heart [6].

Mechanism of action

Monoclonal antibodies act as antagonists by binding to the extracellular alpha-subunit of the receptor, preventing ligand binding and inducing receptor internalization, while small molecule inhibitors compete with ATP at the intracellular beta-subunit tyrosine kinase domain to block downstream signaling cascades [1, 6].
