The Insulin-like growth factor binding protein-like 1 (IGFBPL1) peptide-HLA complex is an intracellular tumor-associated antigen presented on the surface of cancer cells via the Major Histocompatibility Complex (MHC) Class I. While IGFBPL1 is a secreted protein involved in regulating insulin-like growth factor (IGF) signaling and axon growth in the central nervous system, its degradation products are processed and presented as peptides on HLA-A*02:01 in various solid tumors. This complex serves as a highly specific target for T-cell receptor (TCR)-based therapies, such as TCR-engineered T cells (TCR-T), which can recognize and eliminate tumor cells expressing the intracellular protein. Targeting the peptide-HLA complex allows therapeutic access to the intracellular proteome, which is otherwise unreachable by traditional monoclonal antibodies. Current clinical development, such as AFNT-211, focuses on leveraging this target for the treatment of solid tumors where IGFBPL1 is overexpressed.
