Overview

The insulin-producing beta cell of the pancreatic islet is a specialized endocrine cell primarily responsible for the synthesis and secretion of insulin, a peptide hormone critical for the regulation of blood glucose levels. Beta cells are located within the islets of Langerhans in the pancreas and constitute approximately 40–75% of islet cells in humans[1][2][6]. Insulin secretion is triggered mainly by elevated blood glucose and is regulated via an intricate system of nutrient sensing, metabolic signaling, and paracrine communication with neighboring islet cells[3][4]. Beta cell dysfunction or destruction is central to the pathogenesis of both type 1 diabetes (autoimmune-mediated loss) and type 2 diabetes (impaired function and insulin resistance)[3][4][5]. Several classes of antidiabetic drugs target the function or survival of beta cells, including sulfonylureas (stimulate insulin release), GLP-1 receptor agonists, and DPP-4 inhibitors (enhance endogenous insulin secretion via incretin pathways)[4]. Beta cell health can be monitored using C-peptide levels, which reflect endogenous insulin production[4].
