Insulin-reactive B cell receptors are specialized immunoglobulins found on subsets of murine and human B lymphocytes capable of recognizing endogenous insulin. Their engagement leads either toward immune tolerance through editing/anergy or—if these checkpoints fail—to pathogenic activation contributing directly to type 1 diabetes via antigen presentation and subsequent cytotoxic T cell recruitment. The efficiency with which central tolerance eliminates these autoreactive clones varies by genetic background, influencing disease susceptibility.
