Overview

The Integrin–cell adhesion molecule (CAM) interface between progenitor cells and the coronary endothelium is a multi-protein interaction network critical for the homing of regenerative cells to cardiac tissue (PMID: 15604134). This interface primarily consists of integrin heterodimers, such as Integrin alpha-4 beta-1 (VLA-4) and Integrin alpha-L beta-2 (LFA-1), expressed on progenitor cells, which bind to ligands like Vascular Cell Adhesion Molecule-1 (VCAM-1) and Intercellular Adhesion Molecule-1 (ICAM-1) on the coronary vascular lining (UniProt: P13612, P19320). Biologically, these interactions mediate the rolling, firm adhesion, and subsequent transendothelial migration of cells from the circulation into the myocardium. In cardiovascular diseases like myocardial infarction, the expression of CAMs is upregulated by inflammatory cytokines, creating a molecular zip code for progenitor cell recruitment to the site of injury (StatPearls: Cell Adhesion Molecules). While current pharmacological agents like Natalizumab and Vedolizumab are designed to antagonize these interfaces to treat inflammatory conditions, cardiovascular research explores agonists or priming strategies to enhance this interface for improved stem cell therapy. Safety concerns regarding the modulation of this interface include the risk of systemic immunosuppression or the development of progressive multifocal leukoencephalopathy (PML) when using potent antagonists.
