Interleukin-1 receptor antagonist (IL-1Ra) is a naturally occurring protein that serves as a critical endogenous regulator of the inflammatory response. It belongs to the interleukin-1 family and functions by binding with high affinity to the interleukin-1 receptor type I (IL-1RI) without inducing a biological signal, thereby competitively blocking the pro-inflammatory actions of IL-1α and IL-1β (Arend, 2002, Cytokine & Growth Factor Reviews). This inhibition is vital for maintaining immune homeostasis; an imbalance between IL-1 and IL-1Ra is a hallmark of various autoinflammatory and autoimmune conditions, including rheumatoid arthritis and the rare genetic disorder Deficiency of IL-1 Receptor Antagonist (DIRA) (Aksentijevich et al., 2009, NEJM). Therapeutically, recombinant IL-1Ra (Anakinra) is utilized to treat these conditions by neutralizing excessive IL-1 activity (DrugBank, DB00026). Additionally, certain drugs like Diacerein are categorized under this pathway as they exert anti-inflammatory effects by inducing the endogenous production of IL-1Ra and other cytokine modulators to protect joint tissues in osteoarthritis (Martel-Pelletier et al., 2010, Therapeutic Advances in Musculoskeletal Disease).
