The Interleukin-6 (IL-6) and Interleukin-1 beta (IL-1β) signaling axis is a fundamental pathway mediating the inflammatory response in the central nervous system (CNS). This axis is characterized by the synergistic interaction between IL-1β, a potent initiator of the innate immune response, and IL-6, a pleiotropic cytokine that regulates both pro- and anti-inflammatory processes. In neurodegenerative and neuroinflammatory conditions, IL-1β produced by activated microglia via the NLRP3 inflammasome stimulates astrocytes and neurons to secrete IL-6, creating a self-amplifying inflammatory loop that exacerbates tissue damage and increases blood-brain barrier permeability. Therapeutic intervention targeting this axis involves the use of biologics, such as monoclonal antibodies (e.g., tocilizumab) or receptor antagonists (e.g., anakinra), to disrupt cytokine binding and suppress downstream signaling cascades like JAK/STAT3 and NF-κB. While these therapies show promise in mitigating chronic neuroinflammation, they also pose significant risks of systemic immunosuppression and increased susceptibility to opportunistic infections due to the cytokines' essential roles in host defense.
