The intervertebral disc extracellular matrix (IVD ECM) is a complex network of proteins, proteoglycans, and water that provides structural support and mediates cell function within the intervertebral disc. It is composed primarily of collagen types I and II, aggrecan, and other proteoglycans. Degradation of the IVD ECM is a major contributor to intervertebral disc degeneration (IVDD), leading to pain and disability. Therapeutic strategies aim to regenerate the IVD ECM by stimulating endogenous repair mechanisms, delivering exogenous ECM components, and inhibiting matrix degradation.
ECM regeneration aims to restore the structural and functional integrity of the intervertebral disc by promoting proteoglycan and collagen synthesis, inhibiting matrix degradation enzymes, and providing a supportive scaffold for cell attachment and growth.
