Overview

The intervertebral disc extracellular matrix (IVD ECM) is a specialized, non-vascularized structural network that maintains the mechanical function and hydration of the spinal column [PubMed: 25643104]. It is primarily composed of a dense meshwork of Type II collagen fibers and large, sulfonated proteoglycans, most notably aggrecan, which create the osmotic pressure necessary for shock absorption [StatPearls: Intervertebral Disc Degeneration]. In the context of degenerative disc disease (DDD), the matrix undergoes a shift from an anabolic to a catabolic state, characterized by the upregulation of matrix metalloproteinases (MMPs) and ADAMTS enzymes that degrade these structural components [PubMed: 21658561]. Therapeutic interventions targeting the IVD ECM focus on restoring its integrity through the delivery of anabolic growth factors, inhibition of catabolic enzymes, or the use of biomimetic scaffolds. Because the disc is the largest avascular structure in the body, maintaining the health of the matrix is critical for nutrient diffusion and cell viability. Current research focuses on biological agents such as Growth Differentiation Factor 5 (GDF-5) and Link N peptide to stimulate endogenous repair of the matrix [PubMed: 23403567]. These interventions seek to reverse the pathological changes that contribute to chronic low back pain and spinal instability.
