Overview

The intestinal epithelial mucosal surface is a specialized biological interface that separates the internal environment of the body from the external contents of the gut lumen. It is composed of a polarized layer of epithelial cells, including enterocytes, goblet cells, and Paneth cells, which are covered by a protective glycocalyx and mucus layer [PMID: 23524162]. This surface performs the dual role of facilitating the selective absorption of nutrients and water while acting as a formidable barrier against pathogens, toxins, and undigested food particles [PMID: 28123242]. In clinical practice, this surface is the site of action for locally acting therapeutics such as aminosalicylates and corticosteroids used to treat inflammatory bowel diseases (IBD) [StatPearls: Mesalamine]. Pathological breakdown of this barrier, often referred to as leaky gut, is associated with various systemic and localized inflammatory conditions, including celiac disease and Crohn's disease [PMID: 21248165]. Consequently, therapeutic interventions often target the components of this surface, such as tight junction proteins or the mucus layer, to restore homeostasis and prevent chronic immune activation.
