Intestinal fluid secretion via osmotic effect refers to the movement of water from the body into the intestinal lumen, driven by osmotic gradients. These gradients are primarily established by active transmembrane transport of electrolytes—especially chloride ions (Cl⁻)—across intestinal epithelial cells. The process is essential for normal digestion and can be pathologically exaggerated in conditions like secretory diarrhea. The main molecular driver is active chloride secretion by crypt enterocytes in the intestine involving Na⁺/K⁺-ATPase, NKCC1 (Na⁺-K⁺-2Cl⁻ cotransporter), and apical Cl⁻ Channels (e.g., CFTR). When non-permeable or poorly absorbed solutes are present in high concentrations within the gut lumen, an osmotic force draws water out of enterocytes and capillaries into the intestinal space.
