Overview

The intestinal glycocalyx is a dense, carbohydrate-rich layer covering the apical surface of the intestinal epithelium, specifically the microvilli of enterocytes [Biology Online, Wikipedia]. It is composed of a complex network of membrane-bound mucins, proteoglycans such as syndecans, and glycosaminoglycans including heparan sulfate and hyaluronan that form a critical physical and chemical barrier [NIH, MDPI]. This structure serves dual roles: it anchors essential digestive enzymes to facilitate nutrient breakdown and absorption while simultaneously acting as a primary defense mechanism to exclude pathogens and toxins from the systemic circulation [Biology Online, Study.com]. In diseases such as inflammatory bowel disease (IBD), sepsis, and severe acute pancreatitis, the glycocalyx undergoes significant degradation or shedding, which compromises the intestinal barrier and leads to increased permeability and systemic inflammation [NIH, ResearchGate].
