Overview

The intestinal inflammatory cytokine milieu refers to the complex and dynamic network of signaling proteins, including interleukins, interferons, and tumor necrosis factors, that orchestrate the immune response within the gastrointestinal tract (Neurath, M. F. (2014). Nature Reviews Immunology). In a healthy state, a delicate balance between pro-inflammatory and anti-inflammatory cytokines maintains mucosal homeostasis and protects against pathogens. However, in pathological conditions such as Crohn's disease and ulcerative colitis, this balance is severely disrupted, resulting in a self-amplifying cascade of inflammation that leads to mucosal injury and chronic tissue damage (Friedrich, M., et al. (2019). Immunity). This environment is characterized by the overproduction of cytokines like TNF-alpha, IL-12, and IL-23, which drive the recruitment and activation of various immune cells. Therapeutic intervention does not target the milieu as a whole but rather focuses on specific high-impact components within it to restore immune equilibrium. Monoclonal antibodies like infliximab and adalimumab target Tumor Necrosis Factor-alpha (TNF-alpha), while newer agents like ustekinumab and risankizumab target the p40 or p19 subunits of Interleukin-23 (IL-23). Additionally, small molecule inhibitors, such as Janus kinase (JAK) inhibitors, work by blocking the intracellular signaling pathways shared by multiple cytokines within this milieu (Danese, S., et al. (2019). Nature Reviews Gastroenterology & Hepatology). Modulating this milieu is essential for achieving mucosal healing and long-term remission in patients with chronic inflammatory gut diseases.
