Overview

The intestinal lumen and systemic fluid compartments are anatomical and physiological spaces involved in the transit, absorption, and distribution of substances within the body. The intestinal lumen refers to the interior of the gastrointestinal tract, serving as the primary site for nutrient uptake and the localized action of non-absorbable drugs (NCI Thesaurus, 2024). Systemic fluid compartments include the intravascular (plasma) and extravascular (interstitial) spaces that constitute the internal environment where drugs distribute after absorption (Guyton and Hall Textbook of Medical Physiology, 2020). These compartments are fundamental concepts in pharmacokinetics, particularly in compartmental modeling, which describes how drugs move between the gut and the bloodstream (Rang & Dale's Pharmacology, 2019). Because these represent physical volumes rather than specific proteins, enzymes, or receptors, they are not considered molecular therapeutic targets. Instead, they serve as the medium through which drugs reach their actual molecular targets or exert non-specific physical effects, such as sequestration or osmotic shifts (StatPearls, 2023).
