Overview

Intestinal luminal free oxalate refers to the unbound, soluble form of oxalate (oxalic acid anion) present in the lumen of the gastrointestinal tract. This is distinct from oxalate that is complexed with minerals such as calcium or magnesium, which forms insoluble compounds and cannot be absorbed by the intestinal epithelium. Only free (soluble) oxalate can be absorbed through the intestinal wall into systemic circulation; insoluble complexes are excreted in feces. The amount of free oxalate available for absorption depends on several factors including dietary intake and composition, concentrations of calcium, magnesium, phosphate, and other ions, and pH levels at different sites within the intestine. Free luminal oxalate is a key determinant in urinary excretion of oxalate and thus plays a central role in conditions like hyperoxaluria and kidney stone formation. Increased levels or enhanced absorption of intestinal luminal free oxalate contribute to higher urinary excretion (hyperoxaluria), increasing risk for calcium oxalate nephrolithiasis (kidney stones). Enteric hyperoxaluria arises when fat malabsorption leads to more unbound fatty acids binding available calcium; this leaves more "free" soluble oxalate available for absorption.
