Overview

Phospholipids in the intestinal mucus layer, especially phosphatidylcholine, form a hydrophobic barrier on top of the mucus gel that protects the intestinal epithelium from bacteria, toxins, and mechanical injury. In healthy conditions, these lipids prevent bacterial invasion and maintain mucosal integrity. Reduction, degradation, or oxidation of mucus phospholipids (most notably in inflammatory bowel diseases) compromises this barrier, leading to increased gut permeability, dysbiosis, and inflammation. Interventions that restore or mimic mucus phospholipids—such as oral phosphatidylcholine or apoA-I mimetic peptides—are being explored for their therapeutic benefit, particularly in ulcerative colitis. This entry is not a standard drug target (such as a specific receptor or enzyme); rather, it represents a non-protein structural and functional lipid barrier that is essential for intestinal homeostasis and implicated in disease, especially inflammation.
